Material: Thick cardstock (12PT)
Please fill out the form with your pets name that you have chosen, date that you purchased, location where we sent you your pet, and any other helpful information to help us locate your purchase in our records.
Some axolotls re-homed through My Lotl Pets were/ could be ‘PET ONLY’, therefore all of the information can be filled as much as possible and hatch dates can be accurately approximated.
